Steps for submitting your promise
    1. Type your first name in the form field
         "Your First Name".
    2. Type your promise in the "I Promise to…" field.
    3. Download and print one of the
         "I Promise Signs FL".
    4. Take a picture holding the "I Promise FL Sign"
         and upload by clicking on "Browse" to open
         and attach your photo (please limit your image
         (jpg format) size to 1mb).
    5. Read and agree to the terms and conditions
          by checking “I agree.”
    6. Click on "Submit Your Promise" and you
          are done!

* The Florida Department of Health expressly reserves the right to delete, without warning or notice, any image(s) or text deemed to be offensive, inappropriate, or in violation of this notice. Images deemed inappropriate by the department may be replaced with the “I Promise FL” logo.

• I acknowledge that the DOH is the sole owner of all rights in, and to, all uploaded image(s) and text and that it has the right to use or reproduce the resulting image(s) and/or text as often as it finds necessary. The photograph may be used indefinitely by television, radio, newspapers, magazines, newsletters, brochures, Internet, intranet, or in any other media format once released. 
• The DOH is not responsible for the unauthorized reproduction of any image(s) and/or text uploaded to the website www.healthypromisefl.com. The DOH is released from any and all claims of damages or liability of any kind whatsoever related to the image(s) and/or text uploaded.
